A full inquest is expected to take place over the death of a 1-year-old who died at a U.K. hospital, according to a 1 August article published in the BBC.
The inquest stems from the death of Archie Squire, who died from heart failure after repeated visits to the Queen Elizabeth the Queen Mother (QEQM) Hospital in Margate, Kent. A report conducted by the East Kent Hospitals University NHS Foundation Trust noted missed opportunities for appropriate diagnosis, the article states.
Among the issues highlighted in the investigations report included missed opportunities to detect shadowing on chest x-ray imaging by pediatric and radiology teams, incomplete reporting of chest x-ray findings in a timely manner, and inconsistencies in documentation quality. The findings were presented on 1 August at a pre-inquest review.
